Totally Disabled Dependent
Provisions of
the life insurance policy exclude coverage for "totally disabled"
dependents when enrolling in spouse, domestic partner, and dependent
child life insurance. Totally disabled means that an eligible
spouse, domestic partner, or dependent child is:
a.) an inpatient in a hospital, hospice, rehabilitation or convalescence
center, or custodial care facility; or
b.) confined to his or her home under the care of a physician
When enrolling for coverage; coverage will not become effective
until he or she is no longer an inpatient in these facilities or
confined at home. If the dependent was covered under the plan prior
to Jan. 1, 2007, this provision would not apply to coverage in effect
as of Jan. 1, 2007, but will apply to any increase in coverage.
Please note: Dependents over age 19 who are not full-time students
but are covered as a disabled dependent will be required to go through
a review of the dependent's disability by the life insurance carrier
at the time a life claim is filed. In order to be eligible, a child
must be 19 or more years old, primarily supported by the employee,
and incapable of self-sustaining employment by reason of mental
or physical handicap.
If in the event of the death of your dependent, it is determined
by the carrier your dependent was totally disabled when first enrolled,
the life insurance claim may be denied. In this circumstance, no
benefits will be payable, and you will be refunded premium paid.
