How to designate a beneficiary
Click on the first button to the right to obtain instructions on how to update, add, or change a beneficiary in My HUB when you don’t have an enrollment event.
Click on the second button to learn how to designate a beneficiary during an enrollment event in My HUB.
Each life insurance benefit must have its own beneficiary designation that must be selected through My HUB. It is not assumed that your beneficiary designation is the same for each benefit.
When can I change my beneficiary?

When selecting beneficiaries designated as: |

Individuals – each name should be listed as first name, middle initial, last name, i.e Mary A. Doe and not Mrs. M. Doe. Include the address, relationship, social security number and date of birth for each individual. Indicate the percentage assigned to each individual. |

Estates – enter the employee’s name as the beneficiary, select “Estate” as the relationship, and indicate the percentage to be assigned to the estate of the insured. |

Trust – enter the employee’s name as the beneficiary, select “Trust” as the relationship, and indicate the percentage to be assigned to the trust. Please fax a copy of the trust title page to the Employee Service Center at 412-647-7860. |

You may update life insurance beneficiaries at any time through My HUB. When life changes occur (due to the birth of a child, marriage, divorce, death of a beneficiary, etc.) you should review and update your designations.
Who can be a beneficiary?
A beneficiary can be a person, a trust, your estate, organization, etc.
How many beneficiaries can I have?
As many as you want. You must provide identifying details of the individual(s) you wish to select as beneficiaries to clearly avoid future confusion.
What is a primary and secondary/contingent beneficiary?
Benefits are payable to your primary (paid first) beneficiaries.
A secondary/contingent beneficiary is ONLY paid if no primary beneficiary is alive at your death.
