Open Enrollment Checklist
Use the checklist below to help you make your benefits selections:

Review the information about your UPMC benefits for 2010. This information is available when you enroll through My HUB and from the Benefits section of Infonet. |

Contact the medical or dental carriers with specific medical or dental treatment questions by calling UPMC DirectLink at 1-800-994-2752, choose option 2. |

Select your benefits online between Nov. 2 and Nov. 13. Use the Printable
Step-by-Step Enrollment Instructions to guide you.
- Flexible Spending Accounts and PTO Buy do not carry over each year. You must enroll to participate for 2010.
- Protect your loved ones - Life insurance beneficiary designations should be reviewed and updated each year.

Print your Election Summary Statement at the end of your My HUB enrollment. Review it for accuracy and keep it for your records. This is your documentation of what you entered. |

Review information and enroll in the UPMC voluntary benefits by Nov. 30. This information is available at the end of your UPMC benefit enrollment session and through My HUB under My Benefits, My Voluntary benefits.

Complete the MyHealth Requirements. |
|
Send Evidence of Insurability forms (if required) to CIGNA by Dec. 30 if you are electing life insurance for yourself, spouse, or domestic partner. |

Contact the UPMC Health Plan (1-800-994-2752, choose option 2) if you enrolled in the Advantage HMO to elect your PCP after you receive your new UPMC Health Plan ID cards. Staff can search for a PCP online at www.upmchealthplan.com, click on Find a Doctor.

Submit the required dependent documentation by Dec. 15, if enrolling a spouse, domestic partner, or child for whom you are a legal guardian for the first time. You can print forms to submit your documentation during your enrollment through My HUB or from the Benefits section of Infonet.

Submit the required dependent documentation by Dec. 15, for eligible dependent children between age 19 and 25 (enrolled as a full-time student in an accredited school, college, or university). You can print forms to submit your documentation during your enrollment through My HUB or from the Benefits section of Infonet. |

Review your Benefit Confirmation Statement. In early December, your Confirmation Statement will be available online in My HUB and will also be mailed to your home. (Note: This is the final year printed Confirmation Statements will be issued for open enrollment.) Watch for a My HUB alert in early December which will signal your online statement is available. |

Review your first pay advice in January for your benefit deductions. |
My HUB access and log-on issues should be directed to the UPMC Help Desk at 412-647-HELP (4357) or your local ISD help desk. If you need additional help with your benefit elections after reading the materials provided during your enrollment session in My HUB and through the Benefits section of Infonet, contact the UPMC Employee Service Center at 1-800-994-2752, choose option 3.
Benefits described may not be applicable to all staff. Some business units have unique benefit programs, certain job classifications may affect benefit eligibility, and contract terms determine benefits for physicians and members of collective bargaining units.
